It's Wufoo, their UI is delicious! Really, I love their services. But I think their plans are a bit un-proportioned and that's why I didn't pay for one.

yep Sexy ain't it :Simple but professional, but i agree the plans are disproportionate to the prices :/

I actually meant that they give you 10 forms, but only allow 500 submission... so that's like only 50 submissions per form, and that's not enough to gather information.

Not a serious amount no, but it served a decent purpose considering you had just started this up
